How can I create a custom form for my organisation?

Easily build forms tailored to your team’s needs in just a few steps.

We have a large selection of forms that are available to use in our library, but you also have the ability to build your own form.

Have you seen an iAM form you'd like to adapt to suit your suit your specifications? You can also duplicate any form to create an editable copy

Simply navigate to the Forms section and click on 'New' or 'Create a Custom Form' to get started.

Enter the following details:

Form Title – Give your form a clear and descriptive name. If you've got multiple locations in your iAM account and this form will be specific to just one of them, we'd recommend including the location name in the form title.

Description – Provide a brief explanation of the form’s main purpose.

Category – Select the appropriate category for your form. You can choose one of the iAM categories or you could create your own.

On this screen you can also decide if you'd like your form to capture the specific zone or area that the responses to your form relate to, and if you'd like respondents to list any related items from your asset register. Both zone and asset selection will depend on the location that respondents select when filling out the published form.

This information helps ensure your form is easy to find and use within your organisation.

Click Create Form to proceed to the next step where you can start to build your form.

To start adding questions to your form, click 'Edit Elements', and then 'New Element'.

Please note that the Review Form section of this page simply shows a preview of how your respondents will see this form and as such location and asset selection options cannot be made here. 

Choose from the dropdown list of elements listed below depending on the need for the form:

In this example, we have chosen a 'Header Block' element. Selecting 'Save' will save the first section of your form.

Header blocks are a great way of dividing your form into sections. We've also found that the 'Yes/No list' option has been a popular choice for many different use cases.

You're able to add as many elements and questions as you need to build your form. You can also choose to re-order your questions by dragging and dropping them to a new position. Once you're satisfied, click 'Finish editing' to save all your elements and questions.

Before clicking the green 'Publish form' button please review and confirm all your choices as once a form has been published it cannot be changed.
